Abstract: This project reports on the findings of an investigation into reasons why hanging wall examination procedures fail to prevent fall of ground accidents on gold and platinum mines. Ideally, these procedures should identify every unsafe hanging wall condition and ensure that appropriate action is taken to remedy each unsafe condition.
